C-SNPS: Plans for Chronic Living

Certain Medicare Advantage plans are specifically designed for individuals living with certain chronic conditions.

These plans are called Chronic Condition Special Needs Plans, or C-SNPs.

C-SNPs are designed to provide specialized benefits, care coordination, and support focused on improving health outcomes and managing chronic conditions.

Certain chronic conditions that may qualify for a Chronic Plan can include:

  • Diabetes

  • Heart Disease

  • Chronic Obstructive Pulmonary Disease (COPD)

  • Chronic Kidney Disease

  • Cancer

  • Rheumatoid Arthritis and other autoimmune disorders

  • Chronic neurological disorders such as Parkinson’s disease, stroke, or multiple sclerosis

  • Certain chronic mental health conditions

One reason many people choose a C-SNP is because these plans are built specifically around the needs of people managing ongoing health conditions. Some plans may include additional benefits such as:

  • Healthy food or meal benefits

  • Care coordination support

  • Specialized provider teams

  • Personalized care plans

  • Extra help managing medications and appointments

Many Chronic Medicare Advantage plans may also include specialized formularies designed around the medications commonly used to treat chronic conditions.

Did You Know?

Individuals who qualify for a Chronic Plan may be able to make certain coverage changes outside of the standard Annual Enrollment Period depending on their situation.

In some cases, this can help lower prescription costs and improve access to important medications.
