
C-SNPS: Plans for Chronic Living
Medicare Advantage plans are specifically designed for individuals living with certain chronic conditions.
These plans are called Chronic Condition Special Needs Plans, or C-SNPs. According to this month’s presentation, C-SNPs are designed to provide specialized benefits, care coordination, and support focused on improving health outcomes and managing chronic conditions.
